Step 1: Print PBA Medical Form and arrange an appointment to see your GP.

A GP or registered Doctor must complete, sign and stamp the document. Depending on the practise, some will charge (no more than £20) and some will do it for free. This is valid for 12 months.

Step 2: Arrange with an optician to have a standard eye test, this is usually around £20 (depending on optician) but we can give you a free eye test voucher, please contact us for one. This is valid for 12 months.

Step 3: Once the above 2 steps are done, complete PBA Boxer Licence Application.

Step 4: Return all 3 Documents/Forms back to us, either post or email (scan or scan app) along with payment of Licence.
